I have posted the questionnaire for Research Study #2 in two ways:
Excel Spreadsheet file for reference and later use; and
PDF file for printing.
You can try printing both files, but if you have any problems with the Excel spreadsheet file, use the PDF file.
REMINDER: bring in four completed questionnaires, in their sealed envelopes, on Tuesday 3/22/05. TRY to have two of the four completed by someone other than a college student.
